Why automate form
attachments?
Manually attaching forms works fine for occasional use. But when every bug ticket needs the same
severity form, or every "In Review" transition requires a QA checklist, manual attachment creates
friction and gets missed. Automation ensures the right form appears on the right work item — every
time, without anyone having to remember.
Two actions of Smart Forms in Jira automation
Action 1
Attach Smart Form
Attaches one or more forms. Forms open blank — no data is pre-populated. The respondent fills everything in from scratch.
-
Multiple forms at once
-
Forms open blank for the respondent
action 2
Attach Smart form with values via Jira Automation
Attaches a single form and populates specified fields from Jira data automatically. The respondent opens the form with context already filled in.
-
One form at a time
-
Form elements contains values
Why is "Attach with Values" limited to one form?
Each form has its own field structure — field mappings are specific to one form's elements. To pre-fill multiple forms, add a separate action step per form.
Internal vs External — the JSM concept
When a JSM project is in scope, every form selection shows an Internal / External toggle. This
controls which audience sees the form — not what the form contains.
Internal
Shown to Jira agents inside the issue view. Not visible on the Customer Portal.
External
Shown to the customer on the Customer Portal.Use for feedback, NPS, additional info from customers.
