How to Add a Consent Checkbox

  1. Open Smart Forms for Jira and create a new form or edit an existing one.

  2. Add the Consent Checkbox and customize the text (up to 255 characters).

  3. Include a Link to relevant policies (e.g., Terms of Service, Privacy Policy).

  1. Share the Form in Jira issues, the Customer Portal, or externally.

  2. Once Submitted, the form becomes non-editable, preserving the record of the user's agreement.
